Slate had an ok article on the origin of "FAIL!"
http://www.slate.com/id/2202262/
it had a bit too much of "look what I found on wikipedia!" about it, especially in terms of the origin of the FAIL meme as being Blazing Star's "You fail it! Your skill is not enough! See you next time! Bye bye!"

This doesn't sound right to me, but I don't have a N64 or GC handy to test my other theory, but didn't this show up in, maybe like Smash Bros' Homerun Derby? It seems to me the important thing is "FAIL!" as a single word and I thought some old Nintendo game had that...
(kind of like how "___ GET!" might have its origin just in Mario Sunshine screenshots...)

In Smash Bros., the operative term is "FAILURE" delivered by the hilariously serious disembodied announcer voice.

That's what popped into my mind before I got to the part of the post where you brought it up.

Oh, and it started in the "Break the Targets" sections from the n64 game--it's one of the many great signatures of the series.

I thought the origin of "___ GET" was Super Formation Soccer, JP version:

Although the only web page I can find to confirm this isn't a shop job is here.

It's difficult to feel confident in any game of "follow that meme", since it's less of "what appeared first" as "what inspired the core group of meme repeaters"

JP Mario Sunshine seems to be more topical to where it appeared, but I guess if these things burble in a small group for a while and then branch out, anything goes.
